Clinical Pharmacist
Required Education:
· Doctor of Pharmacy or Bachelor's Degree in Pharmacy plus 10 years of relevant clinical experience
Required License:
· Active State Pharmacy License
Preferred Experience:
· Current knowledge and expertise in clinical pharmacology and disease management
- Experience with team-based care.
Preferred Certification: American Pharmacist's Association Medication Therapy Management Certificate Training Program certificate or equivalent.
Preferred Skills/Abilities
- Strong clinical therapeutic skills and demonstration of knowledge of appropriate medication dosing
- Self-management Skills – self-motivated, organized, detail oriented and manages time and resources effectively. Must be able to work independently in a collaborative environment, be self-directed and possess excellent organizational and time management skills
- Knowledge and understanding of the 5 Core Elements of MTM services – 1) Medication Therapy Review – Comprehensive and Targeted, 2) Personal Medication List (PML), 3) Medication-Related Action Plan (MAP), 4) Interventions and Referrals, and 5) Documentation of Services and Follow-up
- Strong Communication Skills – Able to speak clearly and communicate professionally with patients of all ages telephonically. Able to articulate information, both verbally and in writing, in a clear and concise professional and grammatically correct manner with other health professionals. Able to probe and question patients or health care providers to extract information to understand concerns and needs in an efficient manner
- Excellent Interpersonal Skills – demonstrate empathy with callers in tone, language, questions and demonstrate patience, caring, sensitivity, empathy, and cultural competency throughout job responsibilities
- Computer/Technology Competency: Proficient in use of Microsoft Word, Excel, and Outlook
