Under the supervision of the Case Management Manager, the Community Resource Navigator provides targeted outreach to members predicted to be experiencing social determinants of health (SDOH) challenges. This role focuses on assessing psychosocial needs, and coordinating services to address barriers such as housing instability, food insecurity, transportation, and access to care. The “Service Connector” serves as a liaison between members, families, the health plan, and community resources to promote health equity and improve member outcomes.

Key Responsibilities:

  1. Conduct outreach to members with potential SDOH-related needs and perform psychosocial assessments.
  2. Educate members and families on available benefits, programs and community resources.
  3. Coordinate referrals to internal programs and external agencies
  4. Document all interactions and interventions in the managed care system using standardized questionnaires and templates.
  5. Identify community-based services for members and build relationships with local organizations.
  6. Generate internal referrals to clinical team including nurse health coaches and social workers as needed
  7. Perform other duties as assigned.

Qualifications:

  • Education: Bachelor’s degree, preferred in social work, psychology or human services
  • Experience: Minimum of 3-5 years of experience, with a strong focus on addressing social determinants of health and working with vulnerable populations, including seniors
  • Skills:
    • Strong interpersonal, communication, and organizational skills
    • Proficiency in documentation and use of care management systems
    • Knowledge of community resources and social service systems

Fully Remote:
This role is designated by Independence as fully remote. The incumbent will not be required to report to one of Independence’s physical office locations to perform the work. However, the work must be performed in the Tri-State Area of Delaware, New Jersey, or Pennsylvania.
