Director Clinical Quality Outcomes (hybrid)
Summary:
Independence is seeking an Advanced Practice Practitioner (APP), a Nurse Practitioner or Physician Assistant, to join the clinical care transformation team; a team that works directly with healthcare systems and with primary care providers, with a focus in New Jersey. This individual will help drive improvement in quality, experience, health equity, and cost of health care. This work is achieved through collaboration across multiple Independence teams.
Candidates must have a strong sense of accountability, and passion to drive change in the way healthcare is provided. This role will report to the Senior Medical Director in Clinical Care Transformation.
Responsibilities:
The APP will Collaborate with the Independence Population Health Team and with network primary care providers and health systems to:
Drive improved performance in NJ markets for Primary Care including:
· Improved performance in clinical CMS Medicare and ACA STAR ratings
· Engagement with primary care offices and organizations on initiatives which promote accurate medical coding
· Support vendor relationships which drive population health team performance
· Work with on value-based programs with Health Systems
Cross-Functional Teamwork Opportunities:
· This individual will work with Independence Clinical Care Innovation Team, to support pilots and grants which focus on innovative care delivery.
· Work with Independence Provider Experience and Network Operation teams, focusing on enabling existing provider workflows to facilitate and improve the clinical provider experience.
· Work with Independence Director of Health Equity to promote IHG 2025’s whole-person equitable health strategy.
· Additional opportunities to collaborate Case and Condition Management, High Value Optimization, Utilization Management, and Quality
Qualifications:
· AAP must have a valid Nurse Practitioner (NP) or Physician Assistant (PA), unrestricted license to practice medicine in NJ
· Minimum 5 years of experience working for a healthcare: provider (primary care or gerontology), payor or consulting group
· Demonstrated ability to lead through influence to obtain results
· Excellent written and verbal skills and ability to interact effectively with all levels of employees
· Demonstrated effective problem-solving skills and the ability to work in a changing environment
· Experience and ease working in Microsoft Excel, Word, Outlook, and PowerPoint
· Residence in NJ, with a valid driver’s license
Hybrid of Choice:
Independence has implemented a “Hybrid of Choice” model which provides our associates with the flexibility to choose whether to work remotely, work in the office every day, or work in the office on certain days at their discretion. However, management may require our associates to work from Independence’s physical office locations on certain occasions. This role is designated as a role that fits into the “Hybrid of Choice” model. While associates may work remotely, the work must be performed in the Tri-State Area of Delaware, New Jersey or Pennsylvania.
